Deine Aufgaben
- Ansprechperson für Ärzt:innen und Spitäler sowie andere relevante Einrichtungen
- Ansprechperson für Bewohner:innen der Einrichtung im Rahmen des erhöhten Betreuungsbedarfs für Menschen mit psychischen und/oder physischen Erkrankungen
- Beurteilung der selbstständigen Wohnfähigkeit der Bewohner:innen in pflegerischer/medizinischer Hinsicht
- Einschätzung des pflegerischen Hilfebedarfs
- Medizinisch-fachliche Einschätzung des Gesundheitszustands von Bewohner:innen
- Organisation und Kooperation von externen Diensten
- Dokumentation der Betreuung
- Regelmäßiger Austausch mit den anderen Berufsgruppen in der Einrichtung
- Teilnahme an Fallbesprechungen
- Dispensierung und Verabreichung von Medikamenten im Einzelfall und unter Anordnung der behandelnden Ärzt:innen

Deine Aufgaben
- Vorbereitung und Durchführung der standardisierten Auszahlung von Leistungen im Rahmen der Grundversorgung
- Vorbereitung der standardisierten Abrechnung (Erstellung von Belegen und Abrechnungsunterlagen)
- Kommunikation mit interner Administration, Buchhaltung und Fördergeber (über Portal)
- Kassabuchverwaltung
